Provide diabetes education services to Obstetrics patients. Collaborate with RN, MD, and APP’s in regards to appropriate diabetes management and education protocols in the inpatient and outpatient setting. Provide diabetes education class and support group services.

• Collaborate with medical and hospital nursing staff with discharge planning and other patient / family interventions as needed.
• Communicate with patients and families regarding patient progress in clinic settings and via phone call follow- up.
• Develop and deliver requested in-service training and presentations for Transplant teams, Hospital staff, School of Medicine courses, and outside organizations, in regard to: Development and status of program services.
• Facilitate outpatient diabetes classes pertinent to program.
• Facilitate professional development of transplant health care teams (RN, RD, PharmD, and LCSW) by improving clinical theoretical education and knowledge in the field of diabetes. Communication of clinical research protocols, study findings, and current practices.
• Make recommendations involving course of Diabetes management supplies, tools and further education for both the family and patient.
• Monitor patients' diabetes outcomes with out-of-area healthcare team; communicate and document patient progress for transplant physicians and healthcare staff.
• Oversee inpatient care with Transplant teams and hospital staff regarding diabetes management and education, including nursing staff, registered dieticians, and pharmacy services.
• Oversee the multidisciplinary diabetes education team that performs diabetes education / management services for Transplant program.
• Oversight of writing, submission, renewal and revision of IRB protocols and informed consents assuring timely compliance with IRB requirements.
• Prepare manuscripts for publication, in conjunction with research collaborators, for print publications and scientific meeting presentations.
• Provide outreach support to transplant population, including facilitating out of Stanford system referrals to diabetes management teams (physician and staff) local to patient.
• Review and recommend grant proposals, (including developing study design), research methods and study protocols.
Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Education: Bachelor's degree in a work-related discipline/field from an accredited college or university.
Experience: One (1) year of progressively responsible and directly related experience.
License/Certification: Certified Diabetes Educator (CDE)
