The Nurse Coordinator is responsible for providing direct and indirect care to clinic patients and their families utilizing the nursing process - assessment, planning, implementation and continual re assessment. The Nurse Coordinator assists with the provision of patient care. The duties include telephone consultation and education, coordination of care, treatments, medication administration and other duties which are within the scope of practice. Works collaboratively with the health care team - physician, ancillary providers, care centers staff and administration. Responsibilities include coordinating patient contact, patient education, and preparing patients/families for procedures/tests in collaboration with the health care team; collaborating with others to ensure continuity of care and coordination of a wide variety of patient services; identifying opportunities to improve patient care services; assisting clinic management to identify and develop operational needs for the clinic and to improve patient care services; and they may provide direct patient care.

Must perform all duties and responsibilities in accordance with the Service Standards of the Hospital(s).

• Acts as an effective liaison between the patient's provider, Care Center staff, in-patient settings, home, and community.

• Communicates with patients and their relatives to explain diagnostic and therapeutic measures related to condition, provide instruction, and promote participation in patient care. Therapeutically supports the patient/physician relationship; properly coaches, counsels, and prepares the patient for a physician visit.

• Assists clinicians with returning phone calls and calling in prescriptions. Adheres to approved telephone protocols; confers with physician for any illness/injury needing additional telephone advice.

• Conducts telephone interviews and assessments of patients with acute and/or chronic health problems. Communicates with patient walk-ins who have medical questions.

• Consults with the Manager when patient issues or complaints arise pertaining to practice, within one hour of receipt of the complaint.

• Coordinates activities of nursing staff with other personnel and services engaged in caring for patients. Develops and implements processes / procedures to promote effective and efficient use of resources. Assists with implementation of new programs and assists with efforts to promote the service to other health care professionals and to the public. Works collaboratively with clinicians when developing new, and reviewing existing triage protocols.

• Develops and implements follow-up procedures for patient issues or complaints.

• Contributes to interdisciplinary quality review upon request.

• Documents all telephone calls/communications in the patient record within 24 hours of the time of call.

• Surveys and records the client's symptoms, using the nursing process, to attain information on the client's chief complaint, level of urgency, potential of self-care intervention, and type of follow-up needed.

• Collects all demographic and anecdotal data necessary for high quality clinical decision-making. Explains medical terms, diagnostic procedures.

• Provides the proper information regarding prescribed medications and therapies, utilizing appropriate pharmacological references and consultation with the pharmacist or physician.

• Identifies and refers clients with complicated medical histories, health education needs, chronic health problems or treatment compliance to the client's clinician.

• Prioritizes the urgency of the client's medical need and schedules a clinic appointment or connects the client to the local EMS system (911).

• Independently triages and/or advises clients, based on the perceived illness acuity, to the most appropriate level of care (to include the patients’ own physician, clinic or emergency room) utilizing standardized procedures and protocols.

• Schedules or confirms appointments and tests. Answers insurance-related questions accurately or refers to an appropriate resource. Provides information to referrals personnel for authorizations within 24 hours of request.

Minimum Qualifications

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

Education: Graduate of an accredited school of nursing.

Experience: Two (2) years of progressively responsible and directly related work experience.

License/Certification: BLS - Basic Life Support from the American Heart Association. CA Registered Nurse (RN).

Knowledge, Skills, and Abilities

• Knowledge of computer systems and software used in functional area.

• Knowledge of current theories, principles, practices, standards, emerging technologies, techniques and approaches in the nursing profession, and the health care system, and the responsibility and accountability for the outcome of practice.

• Knowledge of laws, rules and regulations; standards and guidelines of certifying and accrediting bodies; hospital and department/unit standards, protocols, policies and procedures governing the provision of nursing care in the area of assignment.

• Knowledge of medical and professional nursing ethics and patient privacy rights.

• Knowledge of medical terminology; principles and practices of health promotion, risk reduction, illness and disease prevention and management; medications and drugs, common dosages, their physical and physiological effects, and possible adverse reactions.

• Knowledge of clinical nursing protocols with emphasis in the areas of disease processes, emergencies, health sciences, and pharmacology.

• Knowledge and understanding of managed care concepts.

• Knowledge of what medical symptomology constitutes urgent/emergent care. Knowledge of medical terminology and abbreviations.

• Knowledge of clinical protocols for vital signs. Knowledge of normal values for vital signs (BP, Pulse, Respirations, Temps).

• Knowledge of medical positioning for patients. Knowledge of scheduling practices/referrals for physician offices preferred.

• Knowledge of effective teamwork practices between front and back office staff.

• Ability to anticipate, assess, plan for, and manage physical, psychological, and social needs of the patient and family/caregiver and provide appropriate support, comfort, and reassurance.

• Ability to apply a professional nursing code of ethics and professional guidelines to clinical practice.

• Ability to develop, evaluate, implement and, as necessary, modify a patient care plan to meet the needs of individual patients.

• Ability to integrate patient care with other members of the interdisciplinary health care team and provide direction or guidance to non-licensed personnel in assigned area of responsibility to ensure quality care.

• Ability to observe changes in the medical condition of patients and effectively communicate these changes to other nursing staff members and physicians.

• Ability to provide age-specific, quality, patient-centered care to all patients through the nursing process and standards of nursing practice with sensitivity and respect for the diversity of human experience.

Pay Range

Compensation is based on the level and requirements of the role.

Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data.

Typically, new team members join at the minimum to mid salary range.

Minimum to Midpoint Range (1.0 FTE): $143,208.00 to $179,254.40
